An AMS provides counseling and education that help provide better patient outcomes. At CHC, we educate patients about blood-thinning medications such as warfarin (Coumadin\u00ae), enoxaparin (Lovenox\u00ae), dabigatran (Pradaxa\u00ae), rivaroxaban (Xarelto\u00ae), apixaban (Eliquis\u00ae) and edoxaban (Savaysa\u00ae).<\/p><h5>What is Anticoagulation?<\/h5><p>Warfarin, also known as Coumadin, is a blood thinner that is used to treat blood clots and lower the chance of blood clots forming or moving. Blood clots can cause stroke, heart attack, or pulmonary emboli (PE). Warfarin is prescribed to those with atrial fibrillation, have had a heart-valve replacement, and to treat patients with venous thromboembolism including Deep vein thrombosis (DVT) and\/or (PE).
